What local services are available to support me?
There are a variety of services within Bradford District & Craven that are available to support people with life limiting illness and their loved ones. Some of these services are listed below. The Healthcare Professionals involved in your care, such as your GP, will be able to discuss these services in more detail with you.
The healthcare professionals and others providing you with support will give you their contact details. If you are concerned or worried please contact them.
If you need specialist palliative care your healthcare team will usually make a referral. If you feel you need this help, please discuss it with your hospital doctor, GP or Clinical Nurse Specialist.

Goldline
GoldLine is a 24 hour a day, 7 day a week service that is available for patients who may be in the last year of their life. It is run by experienced nurses who can provide help and support. This may be in the form of contacting other services on your behalf, such as arranging a District Nurse to visit you at home. If you think you or your relative would like access to the Goldline, please discuss this with your GP or district nursing teams who can refer you if needed.
Carers Resource
The charity Carers’ Resource provides support to anyone who is in a caring role for a loved one. They provide a wide range of different services, including emotional support, advice about finances such as benefit entitlement and support for young carers to name a few. You can contact the charity directly for support, or you can be referred by a healthcare professional.
There are some slight variations between other services depending on location, but your healthcare team will be able to supply you with the information for your local services. Some patients will be eligible for ‘Fast Track’ funding via ‘Continuing Healthcare (CHC)’ when assessed by Health Care Professionals to be in the last weeks of their life. This means that support such as care and equipment can be funded via the healthcare system rather than through the social care system and is not means tested. The process is also much quicker than via the social care route. The eligibility for this is based on a patient’s healthcare needs and their prognosis and includes patients already living in a care home. The healthcare professionals involved in your care, such as GP’s, District Nurses, and Specialist Palliative Care Teams, will be able to talk to you more about whether you are eligible for this.
