If you or a loved one is receiving palliative or end of life care, you may want to start thinking about the future.  There is lots of information available online, from helping you plan how you want to be cared for at the end of your life, to emotional, practical and financial advice and support.

This is a useful resource helping someone to make informed choices, start honest conversations about death and dying with your loved ones, and record and revisit your wishes whenever you want, for free. A living will is a form which lets you refuse any medical treatments that you do not want to be given in the future.
